1.Skilled nursing care services are delivered by registered nurse (R.N.) or licensed practical (vocational) nurse (L.P.N. or L.V.N.).2.Home Health Aide's Services are delivered under the direct supervision or accountability of a licensed nurse (RN/LPN)3.Certified Nursing Assistants (C.N.A.), Certified Medicine Aides (CMA), and Geriatric Nursing Assistants (GNA) provide Home Health Aide services4.An order signed by a doctor is required for all pregnancy-related preventive prenatal and postpartum skilled nursing services.5.No limit is placed on the duration or frequency of home care services. Service frequency and duration must be as put in the patient's plan of care (POC), plan of treatment (POT), or contracted plan of service (POS). Every plan must be reviewed every sixty days for a re-certification or re-assessment.6.All service orders are performed in accordance with accepted standards of medical and nursing practice.Back to our services directory
