Screening, Service Plan and Client Referral - Make Your Own Service Plan
The plan is for clients who live with family members and are partially independent. We provide twenty-four (24) hour service plans 7 days a week (24/7) for the totally dependent individual. To make a service plan:First screen the patient using the "Screening Questionnaire Form"; then go on to the "Service Plan Table" and click the listed service activities for which a Home Health Aide is needed. The "Service Plan Table" includes a table column labeled "Estimated Time for Aide to Accomplish Task". The estimated time approximates how long it will take an Aide to finish a listed task. Just add up the estimated time for the Aide to finish a task to get an estimate of how many hours of service per day that an Aide may be needed. Next, complete the "Referral Form" below, then print the results and submit it to us via mail, or you may contact us directly. A registered nurse (RN) will schedule you for a service assessment and prepare the plan of care.
